Mail Orders: Complete the form below, print a copy and mail to Teen-Aid, Inc. 723 E. Jackson, Spokane, WA 99207-2647.

Payment: Schools should enclose a P.O. number. Individuals can pay by check, money order, VISA, or Mastercard. Do not include credit card information. When your order is received Teen-Aid will contact you for payment information. Statements sent every 30 days. UPS shipping costs and a $5.00 handling charge will be added to all orders. An 8.1% sales tax will be added for Washington state purchases.

Prepaid Orders: The $5.00 handling charge is waived for prepaid orders. Call 1-800-357-2868 for shipping costs. Washington state residents add 8.1% sales tax.

30 Day Preview Period: Teen-Aid offers a 30-day preview for curricula and videos (other materials are non-returnable). Cost of preview: shipping both ways. Returned materials must be in resalable condition and must include payment for shipping and handling charges listed on the invoice.

All Sales Final After 60 Days: No returns accepted after 60 days without written preauthorization.
