Survey of Youth

Instruct the students to wait to complete the survey questions until you have given them the information to complete their codes at the top of the test.

You will need the following information before taking your class to the computer lab

  Your user name (lower case)

  Your password (lower case)

Notes: There are some required fields so if the student does not answer that question it will not allow him to submit his answers. Students must complete the entire test and submit in one session. They cannot do part and return to the form later to finish it.

Remind students to press the Submit button once and return to the desktop of the computer at the end of the session.

Hint: The student can use the Ctrl+F and enter one of the key words the survey is asking to find his place in the test. Once that question is answered. Have him try the Submit function again.
